Your responsibilities

• Provide complete & accurate information about your health, including past & present illnesses, health problems, admission, allergies and current & past medications.
• Read all medical forms including consents thoroughly and clear all your doubts prior to signing.
• Follow the treatment plan recommended by your doctor. Confirm the names of the medicines while buying, and don’t change the brand, dosage or duration of medicines
• Observe facility policies and procedures, including those regarding smoking, noise and visitors. Bring least number of attendants, and avoid bringing along children. Please help us to keep Karthik Netralaya clean.
• To promptly meet all financial obligations.
• To be considerate and respectful of other patients and staff and respect Karthik Netralaya’s equipment & property.
